    HEROIN addicts need more choices to fight the drug, controversial Subiaco-based Fresh Start Recovery Program medical director George O’Neil told State Parliament’s Education and Health Standing Committee last week.

    Dr O’Neil said treatment using addiction-blocking drug naltrezone needed Federal and State government funding because current drug policies only provided the therapeutic drugs methadone or buprenorphine.

    About 50,000 Australians are being treated with methadone at an individual annual cost of about $3000 to prevent HIV infection and stop them committing crimes to feed their habits.

    At Fresh Start, which has an annual budget of $4 million, Dr O’Neil places 100-day naltrexone implants under addicts’ skins using $1.2 million from the State Government, donations and volunteer labour to provide each patient’s $6000 treatment.

    The State Government rejected spending $3m over three years on the program last year.

    Dr O’Neil told the parliamentary inquiry the program provided drugs that blocked addiction instead of switching the dependency to another drug.

    A UK study has just appeared that assesses factors involved with the choice of opiates for maintenance and detoxification. Although both providers and patients are said to contribute to the decision, a stated preference for buprenorphine was more likely to be honored than a preference for methadone. In any event, patients generally rated the medication they received in this study as the “most helpful” in their previous treatment experiences. The authors’ conclusion: “The results . . . suggest that patient preferences and beliefs about opioid agonist treatments can serve as important influences upon clinical prescribing practices.” And indeed, they should; in the absence of data allowing reliable predictions regarding which patients might expect to do better with one or another opioid medication, one must wonder why patient preference is not always respected.
